You would be surprised how often that type of thing happens. I've seen 2 and investigated 6

The riggers using chain instead of FSWR and not using the lifting lugs that are built into the machinery. You can see when they lifted the load the chain was rubbing on a steel edge. And fukc those crazy tag line operators. I think I would have found some longer tag lines!
Wounder if the ship passed its survey after that. May have a bad case of point loading damage. Lucky it landed on the centre line of the vessel.
